単純性腎嚢胞内容液の生化学的検索—特にその発生病理についての考察

はじめに

　単純性あるいは孤立性腎嚢胞（simple or solitarycyst of the kidney）をはじめとして腎の嚢胞性疾患（cystic diseases）の発生病理とか，これに基づく分類は，病理学，X線診断学，泌尿器科臨床上いろいろな解釈や提案が発表されているが，まだ一定していない。その論点の一つに，これらの嚢胞を貯留性嚢胞（retention cyst）と考えるべきか否かという問題がある。

　BrickerとPatton （1955）^1）は嚢胞腎およびいわゆる単純性腎嚢胞の内容液を生化学的に検索して，両者の間に差異のあることを報告し，この違いは嚢胞とネフロンの交通の有無によるものであろうと推論した。著者は単純性腎嚢胞がいかなる分類上の位置を占めるか，さらにその発生病理をどのように考えたら良いか，臨床例における嚢胞内容液の生化学的検索を行ない検討した。

Biochemical investigations were performed on nine simple cyst fluids. The compositions showed more similarity to plasma than urine. The range of urea nitrogen were 15-110mg/dl, creatinine 0.9-14.0 mg/dl, uric acid 2.8-17.3mg/dl, Na 132-164 mEq/L, CI 70-139mEq/L, K 3.6-20. 2mEq/L, Ca 6.5-9.0 mg/dl, Mg 2.0-38mg/dl, P 2.2-260mg/dl, glucose 0-170mg/dl, proteins 0.85-4.9g/dl, Mucoproteins 60- 87mg/dl, and in gel filtration studies high molecular weight proteins (MW, at least more than 2×10⁵) were the major proteins.
