バスケットカテーテルによる抽石術後尿管狭窄をきたした1例

A CASE OF URETERAL STRICTURE FOLLOWING STONE EXTRACTION WITH BASKET CATHETER

はじめに

　尿管結石症に対する治療法は観血的療法と非観血的療法および内科的療法の三者が大別されるが，結石の大きさおよび存在位置により適応を選んでそれぞれ治療されているのが現状である。非観血的療法の1つとして最近一般に普及してきたバスケットカテーテールによる抽出術があるが，これは安易に，また暴力的に行なわれると合併症として尿管損傷などが起こることは衆知のことと思う。ここにわれわれはバスケットカテーテルによる抽石術後，尿管損傷をきたし，その後，炎症性狭窄を起こしたものと考えられる1例を経験したので若干の文献的考察を加え報告する。

Manipulative removal of ureteral stones by basket catheter has become a popular procedure and high successful rate has been obtained by many reporters but not many reports are seen on its compli-cation.

A case of ureteral strictures as a complication of postmanipulative extraction of ureteral stone by basket catheter in a 42 years old male, was reported.

The danger of basket catheter most be well recognized, the absolute indication must be selected and we feel that it must not be used when there is any pathology of the ureter. The meaning of "gentle traction" must be fully recognized and violence should never be employed.
