前立腺摘出術後静脈血栓症の1例

A CASE OF PHLEBOTHROMBOSIS AFTER PROSTATECTOMY

はじめに

　静脈血栓は外傷，感染，妊娠，悪性腫瘍，他の静脈疾患などに際してみられることがあり，また手術，ことに開腹手術後にしばしば発生することが知られている。しかしながら，発生時はほとんど気づかれないことが多く，肺塞栓を起して始めて発症が想像されたり，手術の影響が回復して歩行しはじめてから苦痛に気づいたりすることが多く，その診断も実際にはなかなか難しいようである。諸外国においてはその発生頻度はかなり高く，その発生からさらに肺塞栓症を惹起する危険があるとして極めて重大な疾患であるという。ところが本邦においては本症の発生は少なく，その合併症てある肺塞栓症あるいはPostphlebitic syn-dromeも少ない。泌尿器科領域においても本症の報告は極めて少ないので，われわれが経験した症例を報告し，若干の検討を試みた。

A 63-year-old man was admitted for acute urinary retention. The diagnosis of benign prostatic hyperplasia was made by rectal and urethrographic examination.

Blood chemistry revealed no abnormality. Bleeding time and blood clotting time were within normal limits.

Suprapubicp rostatectomy was done. The operation time was 2 hours and the amount of bleeding was 900 g. The swelling of the left leg measuring 52 cm in circumference, with pain was found 3 days after operation. Phlebography of the left leg was carried out 15 days after operation, and the diagnosis of phlebothrombosis was made.
