尿中β-Glucuronidase測定による職業性膀胱癌の早期診断の検討

Ⅰ．緒言

　癌の臨床における早期診断と早期治療の重要性はよく認められている。しかし初期癌は無症候性であるために，早期診断は重要であるにもかかわらず，実際には極めて困難である。膀胱癌においても早期診断の重要性と困難性とは癌全般についてと同じである。すなわち膀胱癌は膀胱鏡検査により的確に診断されるが，その時にはもはや早期診断とはいい難いほどに癌が進行していることが多い。したがつて膀胱癌の早期診断には多人数の中から膀胱鏡検査をうけるべき人を選別できるようなScreening testが確立されねばならぬ。

　膀胱癌は職業癌の代表的なものであり，わが国で確認されている唯一の職業癌である。すなわち膀胱癌は染料化学工場従業員という比較的限定された職業集団に多発することが知られている^1）。したがつてその早期診断は産業衛生上，労務管理上の重要課題であり，その確立には多大の努力が払われてきた。その結果現在では尿の細胞診による早期診断法が確立している^3）。

After basic investigation on chemical assay of urinary β-glucuronidase, a modified method of Tala lay et al.'s was established. A weak point common to all methods based on Talalay et al.'s was excess amount of urine compared with P.M.G.. Volume of urine in estimation had to be less than twofold of P.M.G. volume.

Activity of β-glucuronidase in urine do not change or decrease for 17 days, if the samples are stored between 0-5℃.

Consecutive studies of urine in 77 cases, engaged in chemical industry, were done by microscopical examination with Papanicolaou's staining and chemical assay of β-glucuronidase at the same time.
