後腹膜脂肪肉腫の1例

A CASE OF LIPOSARCOMA

Ⅰ．緒言

　後腹膜腫瘍症例は近年比較的頻繁に報告されている。後腹膜腫瘍は副腎外褐色細胞腫を除き，ほとんどの場合，腫瘍それ自体の症状を欠き，著しく増殖してのちはじめて，腹部膨満，腫瘤触知，等により発見されることが多い。したがつてその発生より来院までの期間の長いことが，治療上の一つの問題点ともなつている。

　後腹膜腫瘍は後腹膜腔の諸組織を発生母地とするため，その種類は多い。それら諸腫瘍のうち，脂肪肉腫に関して，本邦と欧米ではその頻度において大きな差がみられる。すなわち欧米では後腹膜腫瘍のうち，脂肪肉腫は最も頻度が高いとされているが^1），本邦では極めて少数のものが報告されているにすぎない^2）。

Case is a 55 years old male clerk visited our clinic with the chief camplaint of abdominal mass and emaciation but nothing was particular in his past and family history. Present illness started about 6 months ago when he felt a hen egg sized abdominal tumor which gradually developed to a child head size. The diagnosis of retroperitoneal tumor was established after various examination and surgery was performed. The tumor occupied the entire right retroperitoneal space with adhesions between the upper pole of the tumor and diaphragm, displacing the right kidney to the left crossing the spine, by surgery.
