異食症に伴う巨大な便塊に惹起された尿閉

A case report of urinary retention caused by huge retained feces which is due to allotriophagy

症例は32歳男性，尿閉を主訴に受診，精神発達遅滞，異食症，てんかんを合併していた。腹部CTでは直腸内に巨大な便塊と異物を認め，膀胱は圧排され右側方に強く偏位していた。下剤，浣腸により多量に排便，その後の腹部CTでは便塊は消失し，膀胱圧迫所見も消失，自排尿可能となった。本症例は直腸内の巨大な便塊が膀胱を圧排することで尿閉をきたしたと推測された。

A 32-year-old-man with mental retardation, allotriophagy and epilepsy visited our hospital with a complaint of urinary retention. Abdominal computed tomography showed huge feces, foreign substance in the rectum and the urinary bladder depressed to the right-side. After defecation by the use of a purgative and an enema, the feces was removed and the urinary bladder recovered from the deformation. He restored voluntary urination. This case suggested huge feces can cause urinary retention.
