メスナ，アドリアマイシン，イホスファマイドおよびダカルバジン併用療法が奏効した再発性後腹膜平滑筋肉腫

The recurrent retroperitoneal leiomyosarcoma that responded to Mesna,Adriamycin,Ifosfamide and Dacarbazin combination therapy

症例は68歳女性。腹部膨満感を主訴に近医を受診した。後腹膜腫瘍を指摘され，当科を紹介された。後腹膜腫瘍摘除術を行い，平滑筋肉腫と診断した。術後3か月目に，下大静脈進展を伴う局所再発および肺転移を認め再入院した。CYVADIC（CPA，VCR，ADM，DTIC）療法が無効だったが，MAID（Mesna，ADM，IFM，DTIC）療法により，部分寛解を得，現在も継続中である。後腹膜平滑筋肉腫に標準化された化学療法はなく，本症例はMAID療法が奏効した本邦第1例目の報告と思われる。

A 68-years-old woman with enlarged abdomen was diagnosed as a retroperitoneal tumor by nearby doctors and introduced to our　department. We underwent retroperitoneal tumor extirpation,27×18 cm in size and 4,900 g in weight,and histopathological diagnosis was leiomyosarcoma. She was rehospitalized because of local recurrence with the inferior vena cava extension and metastases to lung three months after the operation. MAID（Mesna,ADM,IFM,DTIC）therapy,followed by CYVADIC（CPA,VCR,ADM,DTIC）therapy resulted partial remission. There is no standardized chemotherapy for a retroperitoneum leiomyosarcoma,and this seems to be a first case of successful MAID in Japan.
