小腸出血に対するアルゴンプラズマ凝固法が原因と思われる膀胱粘膜下腫瘍

A case of submucosal tumor caused by endoscopic coagulation treatment for intestinal bleeding

　血尿と排尿時違和感を主訴に前医を受診し，尿膜管癌の疑いにて手術目的で当科を紹介され受診した。精査の結果，血尿発症の前日に施行された小腸内視鏡によるアルゴンプラズマ凝固法時に膀胱に直接凝固エネルギーが伝わったことにより，炎症性の粘膜下腫瘍が形成された可能性が疑われた。併発していた小腸腫瘍に対し，後に行われた摘出手術時の腹腔内所見も，炎症性粘膜下腫瘍を裏付けるものと考えられた。他科における治療，処置後の合併症としては極めて稀な症例であったので報告する。

A 65-year-old man complaining gross hematuria and bladder irritability, was diagnosed as possible uracal tumor by CT and MRI. We intended radical cystectomy, however the size of tumor decreased after three months of conservative follow up. He had received endoscopic coagulation treatment for intestinal bleeding before he realized gross hematuria. We therefore suspected submucosal bladder tumor formation as an iatorogenic complication. Iatrogenic bladder tumor as seen in this case is infrequent and interesting.
