Two patients with generalized psoriasis were treated with 0.025% fluocinolone acetonide cream, 20 gm/day (equivalent with 5 mgm fluocinolone), applied by ODT over a 2 to 3-week period during which time they were determined every day for serum 11-0HCS and urinary 17-OHCS and 17-KS levels. With this treatment both serum 11-0HCS and urinary 17-OHCS showed a definite declaining tendency for initial 3 to 7 days and then began to increase with marked improvement of eruptions to return to pretreatment levels at termination of the treatment period. These results may be interpreted as indicating the possibility that in psoriatic patients with fairly extensive skin lesions having impaired barrier zone the use of 0.025% fluocinolone acetonide cream under ODT even in ordinary therapeutic dose of 20 gm/day (5 mgm fluocinolone) gives rise to the percutaneous absorption of fluocinolone ace-tonide in an amount adequate to cause adrenocortical suppression. Adrenocortical suppression thus produced, however, was transient in duration and reversed promptly on cessation of ODT after ac-complishment of improvement of eruptions.
