結節性黄色腫（Familial Type Ⅱ）

XANTHOMA TUBEROSUM: FAMILIAL TYPE Ⅱ

　黄色腫とコレステロールの関係は，すでに1878年にQuinquandが3例の患者血液中にCholesterinの増加を確認しているが，病因としてコレステロールをとりあげたのは，これより約30年遅れたPinkus and Pickの報告である^1）。その後Thannhauser，Lever^2）の黄色腫ないしその周辺の疾患を高脂血症の立場から整理した分類は著名である。さらに近年，高脂血症は，コレステロール，トリグリセライドなど脂質そのものより，その転送に働くリポ蛋白の側から検討されるようになり，1967年Fredrickson^3）は高脂血症をⅠ型からⅤ型の5つの型に分類した。著者らは結節性黄色腫のリポ蛋白分析によりFamilial Type Ⅱと診断した症例を紹介し，その概要ならびに成因について述べてみたい。

　Case 1: A 53-year-old man with xanthoma palpebrae and xanthoma tuberosum on the cubital and patellar regions, in the Achilles' tendons, and on the lateral areas of both toes was reported. Serum total cholesterol level was 407 mg/dl, and lipoprotein SfO-10 was 254 mg/dl. The findings of pulse wave of peripheral arteries showed the picture of arteriosclerosis.

　Case 2: A 49-year-old man, a younger brother of case 1, had xanthomatous lesions on the eyelids and patellar regions. Serum total cholesterol level was 342mg/dl and lipoprotein SfO-10 was 314mg/dl.

　Hypercholesteremia was proved in 6 out of 16 members of their family.
