I.はじめに
環状肉芽腫Granuloma annulareは1895年,Fox1)によりUnusual ringed eruption on the fingersとして記載されたのが最初とされる。次いで1902年,Crocker2)が初めて,Granuloma annulareと命名した。さらに1908年,Little3)が経験例を含めた49例について総括的記載を行なつた。以後,欧米においては多数の報告があり,本症の臨床像,組織像,発症病理,鑑別診断,治療などにつき色々と論じられている。種々のSynonymがあるが4)102),一般にはこの病名が使用されている。本邦では,昭和6年の百瀬5)の報告を第1例とし,戦前は稀な疾患とされていたが,戦後は報告例も増加し,最近では,それほど稀な疾患ではなくなつた感がある。著者の集め得た範囲の本邦報告例5)〜98)は,43年3月までに120例を数える。最近経験した3例を報告し,本邦例を総括した要点を記載して,本邦における病像確立の一端に資したい。
Three cases of this disease were reported.
Case 1 : A 1-year-and 4-months-old girl had had ten annular lesions without subjective sensation on the right upper extremity, both lower extremities, and left ankle since 1 year of age. Two months after biopsy they cured spontaneously. No recurrence was noted for one and a half year.
Histologic specimen showed mucin deposit but no giant cell.
Case 2 : A 42-year-old woman had a typical lesion on the back of the right third finger. It disappeared 2 months after biopsy, but recurred 7 months later. It was treated by local injection of steroids.
Histologic specimen showed deposition of mucin but no giant cell.
Case 3 : A 71-year-old man had 4 lesions with slight tenderness on the back of both second fingers. It cured by 3 months of V.E. administration after biopsy. There was no recurrence for 3 months.
Histologic specimen had giant cell, but no deposition of mucin.
Review of 112 reported cases in Japan including the author's cases was performed.
