全身性エリテマトーデスと帯状疱疹

SYSTEMIC LUPUS ERYTHEMATOSUS ACCOMPANIED BY HERPES ZOSTER

I．はじめに

　全身性エリテマトーデス（以下SLEと略す）に帯状疱疹（以下HZと略す）が合併しやすいことはTuffanelli　＆　Dubois^1a），Vachtenheim^2），浜口^3）らによつて指摘されている。そしてVach-tenheim^2），浜口^3）はその原因をSLEにおける免疫系の異常に帰している。著者らも最近，2例のSLEにHZが合併し，うち1例がHZの再発を来した例を経験したので（昭和42年，東日本連合地方会にて発表），あらためて当教室の最近11年間のSLE患者についてHZ合併の面より統計的観察をなし，SLEにかなり高率にHZが合併することを確認した。今回はこの統計的観察について述べるとともに，SLEにおけるHZ合併の問題を，HZの発症機転，SLEの免疫系の異常，ステロイドの影響などの面から検討してみた。

Rather high coincidence of HZ and SLE has been noted by many authors, cause of which should be analysed from the stand points of immunoglobulin, neutralizing antibody, cellular immunity, properdin and interferon.

The authors had 6 (22.2%) of HZ in 27 cases of SLE for 11 years (1957-1967), including 2 cases of recurrent type. It should be emphasized that 4 in 6 cases were teenagers.

From the results of clinical findings and laboratory tests, these cases belonged to mild or severe SLE. However occurrence of HZ is not necessarily the sign of evil prognosis, because all cases in this report are alive, including 2 cases of 10 years' history.

Analysis of the authors' cases suggested that the abnormality in immunological system in SLE as well as steroid therapy might induce HZ.
