Keratoacanthomaの1例

KERATOACANTHOMA

I．はじめに

　本症についての最初の記載は1889年にJona-than Hutchinsonが"Crateriform ulcer of theskin"と題して発表した症例であるとされている^1）。続いて1893年にはLassarが，又，今世紀に入つてからはGougerot（1917），Dupont（1930）などが本症と思われる症例を夫々報告したが，本症が一般に注目されるようになつたのは1936年MacCormac　＆　Scarffがmolluscumsebaceumと名づけて本症の10例を一括報告してからであると云われている^2）。それ以来30年間に欧米では頗る多数の報告例があり最早珍らしい疾患ではなくなつた。

　他方，我国に於いては古谷等^3）が第1例を報告した昭和32年から今日までの10年間に可成りの症例が報告され，それらは文献上少なくとも60余例に達するが尚現在でも比較的珍らしい疾患に属している。殊に本邦例では顔面，頭部，頸部など所謂首から上の露出部位に生じたものが殆んどであつて，その他の部位に生じたものも報告されてはいるが稀である。

A 73-year-old man found a verrucous tumor without subjective sensations on the back of his left hand 2 months ago. It has enlarged rapidly since about 10 days.

At his first visit, it was an elastic, hard, hemispherical nodule, 1.5×1.7×0.5 cm. in size, which in its center had a crater with a horny plug-like, grayish-white scale. There was neither infiltration or induration around the tumor, nor was regional lymph node swelling. Its histologic picture was typical of keratoacanthoma.

A review of localized keratoacanthoma reported in Japan for the last 10 years especially on the sex, age of onset, localization, size and clinical course was performed.
