Testicular cancers are the most common malig-nancies in young men, and the presence of neck metastasis is a rare but well-established phenome-non. A 23-year-old man presented rapidly enlarging bilateral neck lymphadenopathies, which were his-tologically interpreted as teratocarcinoma. To resect multiple metastatic lesions including of neck masses, surgical resection consisted of retroperi-toneal lymph node dissection, thoracotomy and neck dissection were performed after cisplatin-based chemotherapy. There is no evidence of recurrence at the time of this report. We concluded that aggres-sive resection of disease is beneficial for the management of progressive testicular cancer for improving prognosis and quality of life.
