椎骨動脈解離が原因と考えられる突発難聴の1症例

Unilateral Sudden Hearing Loss due to Vertebral Artery Dissection:A Case Report

　はじめに

　突発難聴は原因不明とされるが，治療により聴力回復を示すことから，その原因の1つに血流障害が考えられている^1）。それらの多くは血管条を中心とした迷路性の循環障害を想定したものであるが，椎骨脳底動脈循環不全のような後迷路性の脳血管障害に起因する報告例も散見されるようになってきた^1〜3）。

　今回われわれは，回転性めまいを伴う突発難聴で発症し，画像診断で椎骨動脈解離が原因と考えられた症例を経験したので報告する。

A 58-year-old male presented with a sudden sensorineural hearing loss of the right ear and ver-tigo due to unilateral vertebral artery dissection. Audiogram showed a moderate sensorineural hear-ing loss of the right ear. Spontaneous nystagmus toward the right side was found. MR angiography and conventional angiography revealed the right vertebral artery occlusion. This case elucidated that a cerebrovascular accident could cause a sudden sensorineural hearing loss.
