犬吠えによる急性音響外傷の2症例

Two Cases of Acute Acoustic Trauma Induced by Dog Bark

　はじめに

　強大音の曝露が聴器に障害を及ぼして難聴を生じることはよく知られた事実である。音響曝露による急激に発症する難聴を詳しく検討すると，急性音響外傷，急性音響難聴と騒音性突発性難聴の3つに分類できる^1）。第1の急性音響外傷は「予期しない突発的な強大音によって生じる急性の聴覚障害であり事故あるいは災害に相当するもの」と定義されている^1）。そしてこの事故・災害をもたらす原因音についての症例を集計した研究があり^2,3），日常の社会生活の中で原因音が身近に存在することが示唆されている。

　今回われわれは，ペットの飼犬の吠え声によって生じたと考えられる急性音響外傷の2症例を経験したので，これらの臨床像を記載するとともに，文献を引用しながら2〜3の点について考察を加えて報告する。

A 56-year-old woman was complaining with hearing loss and tinnitus, which were occurred im-mediately after a dog barked near her left ear. Audiogram examinations showed a sensorineural hearing loss of high tone area. The second case was a 69-year-old man presented with left hearing impairment and tinnitus, which were occurred after his pet dog barked near his left ear. An audiogram reviwed the low tone type of sensorineural hearing loss.

Diagnosis of an acute acostic trauma by a dog bark was made.
