感音性難聴を伴った急性中耳炎症例の検討

Acute Otitis Media Associated with Sensorineural Hearing Loss

　はじめに

　急性中耳炎は，日常耳鼻咽喉科診療において比較的頻度の高い疾患であり，特に市中の診療所においては最も頻繁に遭遇する疾患の1つである。急性中耳炎により引き起こされる難聴は，伝音性難聴であることがほとんどであるが，本邦では1953年の切替ら^1）の報告以来，急性中耳炎に内耳障害を合併した症例の報告が散見されるようになってきた^2〜10）。今回当診療所で経験した感音性難聴を伴った急性中耳炎症例について，代表的な1症例を呈示するとともに全8症例の臨床経過をまとめ，内耳障害の原因について文献的に検討したので報告する。

Six patients (6 ears) with sensorineural hearing loss associated with acute otitis media were evaluat-ed. The patients were 16 to 37 years old (average 30.2 years old). Five of 6 patients complained ofotalgia and 4 of them experienced autophonia and hearing loss. One patient complained of dizziness, but nystagmus was not be recognized. Four patients showed hyperemia of the tympanic membrane under otoscopic examination, but no bullous myringitis was observed. They were treated by systemic administration of antibiotics, steroids and vitamins, and all patients showed recovery of hearing and dizziness.
