口腔腫瘍切除例における2層性人工粘膜の使用経験

Clinical Evaluation of Bilayer Artificial Mucous Membrane in Oral Surgery

　はじめに

　舌をはじめ口腔の良性腫瘍を切除する場合，よほど大きくなければ切除創部は一次縫合され，術後嚥下，咀嚼，発声には支障をきたさないのが普通である。しかし悪性腫瘍の場合には，舌部分切除の程度であれば一次縫合されるが，舌の半切以上になると遊離の前腕皮弁や腹直筋，有茎の大胸筋皮弁などが使用されるようになってきた。舌の半切までの切除範囲の場合，一次縫合することで手術時間は短縮されるが，術後の患者の口腔内咀嚼能を経過を追って観察していると，瘢痕収縮のためか，かなり舌の運動が制限され長い間不都合を訴える患者に遭遇することもある。そこで，今回，人工皮膚あるいは人工粘膜としての効果が期待され開発された^1〜3）素材を舌腫瘍，口腔底腫瘍切除後の創部に使用し比較的良好な結果を得たので報告する。

We applied a new bilayer artificial mucous mem-brane (atelo-collagen) to a mucosal defect of 6 patients with cancer of the oral cavity (5: tongue, 1: oral floor). The material was found to be easy to handle, adhere well and promote hemostasis, relieve pain, limit infection, promote rapid epithelization with minimum contraction. The results suggest the safety and effectiveness of the material for clinical use.
