喘鳴を主訴としたTracheopathia Osteochondroplasticaの1症例

A Case of Tracheopathia Osteochodroplastica with Stridor

　はじめに

　Tracheopathia osteochondroplastica，あるいはTracheobronchopathia osteochondroplasti-caは気管および気管支に骨ないしは軟骨組織が異所性に増生し，内腔の結節状隆起を示す良性疾患である。本症は1857年にWilks^1）が報告して以来，外国では約300例，わが国では1938年の城所ら^2）の報告を初めとして60例余りの報告があるが^3），特異的な症状はなく，剖検時^4,5）や全身麻酔時の挿管困難6）によって発見された例も報告されている。

　今回，われわれは喘鳴を主訴としたTrache-opathia osteochondroplasticaの1例を経験したので，若干の文献的考察を加え，報告する。

A 69-year-old female complained of stridor. Bronchofiberscopic examination revealed a bos-selated lesion occupying the anterolateral wall of the trachea just beneath the larynx. Biopsy was performed after tracheotomy. Histological exami-nation showed hyperplastic cartilagenous tissue with ossification. After the biopsy, the lesion was removed as much as possible with forceps. Tra-cheostoma was closed two weeks after surgery and the patient remained asymptomatic.
