嚥下困難を主訴とした下咽頭の異所性扁桃の1例

Ectopic Tonsils in Hypopharynx Causing Dysphagia:Report of a Case

　はじめに

　ヒトの本来の扁桃はWaldayerの咽頭輪と呼ばれ，咽頭扁桃，耳管扁桃，口蓋扁桃，舌根扁桃などからなる。これらと異なる部位にある扁桃を異所性扁桃というが，以前よりいくつか報告されている^1〜5）。今回われわれも下咽頭に発生した異所性偏桃の症例を経験したので報告する。さらに異所性扁桃について，その定義および発生の経緯につき，若干の文献的な考察を加えて報告する。

A 37-year-old male complained of dysphagia most likely due to inflammation of ectopic tonsils in the hypopharynx. The ectopic tonsil in this case met 4 criteria that Fioretti had suggested. Some of 4 cases of ectopic tonsils, previously reported, how-ever, did not meet the 4 criteria. The ectopic tonsils of the 4 cases including the present case were thought to develop from the pharyngeal arches. It was not clear whether ectopic tonsils developed only from pharyngeal arches or pouches.
