異所性甲状腺に発生した濾胞癌の1例

Follicular Carcinoma in Ectopic Thyroid:Report of a Case

　はじめに

　甲状腺は胎生第3週に無対舌結節とコプラとの間に発生し，咽頭腸の前を下降して第7週には気管前壁に到達する。この下降に際し，下降不全が生じたり，下降の途中で甲状腺組織の一部を残したりする。この残存組織が異所性甲状腺となる。思春期の代償性腫大や腫瘍が発生すれば症状も発現するが，多くは一生無症状に経過する。

　今回われわれは，固有位置に甲状腺をもち，甲状舌管を伴わない異所性甲状腺より発生した濾胞癌の1例を経験したので報告する。また，甲状舌管を伴わない異所性甲状腺より発生した癌の報告は少なく，今まで約30例を数えるのみである。これらのうち組織型が判明した症例を集計し，考察を加えて報告する。

A 74-year-old male presented with follicular carcinoma originated from an ectonic thyroid with-out thyroglossal duct in the thyroid cartilage. This disease is rare and approximately 30 cases have been reported in the literature.

A statistical analysis of 13 patients with this disease revealed that the follicular type was domi-nant before 1988, but the incidence of the papillary type has increased since 1988.
