頭頸部領域の神経鞘腫の臨床的検討—埼玉県立がんセンター17年間の経験

Schwannomas in the Head and Neck

　はじめに

　神経鞘腫は神経鞘（Schwann cell）から発生する良性腫瘍である。頭頸部領域での発生は全身のうち25〜45％と比較的多く^1〜3），近年は報告例も増えている。これらの文献を検討すると次のような問題点があげられる。すなわち術前に神経症状を呈することの少ない良性腫瘍であるが術後には起源神経の脱落症状をきたすことがあること，また術前に組織診断が困難なことが多いことなどである。

　そこで神経鞘腫の臨床像を明らかにするために，当施設での頭頸部領域における症例を検討した。

Twenty-three patients with schwannomas in the head and neck were operated on at the Saitama Cancer Center during the period from 1975 to 1991. The distinctive clinical features of schwannoma at different anatomical site were studied.

In the lateral neck, parapharyngeal space and parotid glands, it was difficult to diagnose the tumors as schwannoma before the operation. Eleven cases showed paralysis of the affected nerve after surgery.

In 6 cases of the tongue and the nose, the nerve of origin was not identified and there was no neurological deficit.
