術後の咽頭皮膚瘻（唾液瘻）に対するD-P皮弁を利用した一期的閉鎖法

Deltopectal Flap for One-stage Reconstruction of Pharyngocutaneus Fistula following Total Laryngectomy

　はじめに

　喉頭癌や下咽頭癌の術後などに咽頭皮膚瘻を形成し唾液瘻の発生に悩まされることがある。その際の咽頭皮膚瘻の閉鎖法にはいくつかの方法があるが，今回われわれはD-P皮弁を用い，皮弁の先端部に島状皮弁を作成し折り返すことにより一期的に粘膜欠損と皮膚欠損を修復する方法を経験したので，その概要につき報告する。

A postoperative pharyngocutaneous fistula is a troublesome complication following total laryn-gectomy.

In order to manage this complications, one-stage repair of postoperative pharyngocutaneous fisutula using a deltopectal flap was used, and both skin and pharyngeal defects were immediately covered.

We utilized this method in 5 cases and all patients had restored successful swallowing func-tion.
