CO2 Laser Surgery for Nasal Turbinates:Indication and Effectiveness

　緒言

　下甲介手術におけるLaser Surgeryの応用はまだ歴史が浅く，1977年Lenz^1）のArレーザーによる報告から10年余といったところであるが，この間各施設でさまざまな改良が加えられ，かなり完成された形で現在臨床化されている。本邦ではCO₂レーザーと接触型Nd：YAG （以下，YAGと略）レーザーが主流になっているが，CO₂レーザーは粘膜表層のみを蒸散させる下甲介焼灼術であるのに対し，接触型YAGレーザーは下甲介切除術に近い組織深達性があり多少趣を異にする。すなわちCO₂レーザー光はその性質上吸収率が高く，また反射散乱率も低く，色素選択性がないため^2），鼻アレルギーなどの疾患において鼻粘膜表層のみを効率よく安全に焼灼するのに適しているといえる。今回われわれは難治性の鼻閉疾患としてハウスダスト（以下，HDと略）アレルギーおよび，血管運動性鼻炎，花粉症，肥厚性鼻炎症例に対してCO₂レーザー下甲介手術を施行し，その適応と有効性について検討した。

The carbon dioxide laser was applied to 80 patients with nasal obstruction. The subjects were 61 cases with nasal allergy due to house dust, 6 with vasomotor rhinitis, 4 with Japanese ceder pollinosis with perenial nasal obstruction and 9 with chronic, hypertrophic rhinitis.

The entire inferior turbinates were photocoagu-lated with a defocused laser beam at 8~22 W under local anesthesia.

The laser treatment was effective in 68 out of 80 patients in treating nasal stuffiness. Thirty four out of 56 patients were relieved from sneezing and 33 out of 61 patients were free from nasal discharge.

Effective rate in nasal allergy due to house dust was 78.7%, vasomotor rhinitis was 66.6%, Japanese ceder pollinosis was 75.0% and chronic, hypertrophic rhinitis was 88.9%, respectively.

Recurrence of symptoms was found only in 16 cases 4 to 12 months after the treatment.

In conclusion, CO₂ laser surgry on the inferior turbinate is an effective treatment for various nasal diseases with obstruction. This is a recom-mendable office procedure because of its simpliciy.
