Sweep Frequency Tympanometryで予測できた先天性耳小骨離断の1例

A Case of Congenital Ossicular Dislocation Diagnosed by a Sweep Frequency Tympanometry

　はじめに

　現在チンパノメトリーは，滲出性中耳炎や耳管狭窄症の診断に有効な手段として，広く普及している。しかし，本法は外耳道圧変化に伴う中耳のインピーダンス変化を測定するものであり，プロープ音が220Hzと低いため，中耳インピーダンスのうち，主にスチフネスリアクタンス変化が反映される。このため，中耳病変の中でも主に，マスリアクタンス成分である耳小骨病変の診断には不十分である^1,2）。今回われわれは，通常のインピーダンスオージオメトリを改良したSweep Fre-quency Tympanometryを使用し，耳小骨離断との診断のもとに手術を行い，術後聴力改善が得られ，術後のSweep Frequency Tympanogramでも改善を認めた1例を経験したので，ここに報告する。

A 19-year-old female complained of a non-progressive hearing loss in the left ear. Otoscopy revealed no abnormal finding in the external car and the ear drum. Audiometry showed 48 dB A-B gap. The tympanogram was A-type with no stapedial reflex. It was impossible to determine the pathology of the ossicular chain by target CT. By the sweep frequency tympanometry, ossi-cular dislocation was strongly suggested because of abnormal values of B-, Z- and P-points. This was confirmed by surgery. The hearing and the sweep frequency tympanogram improved up to normal range after the operation.

The authors feel that the sweep frequency tympanogram is useful for the diagnosis and the preoperative preparation in ossicular abnormalities.
