A 55-year-old woman developed bilateral oculomotor palsy after blunt head trauma during a traffic accident. She underwent neurological exami-nations and other supplementary studies including computed tomography, magnetic resonance imag-ing, auditory brainstem response and short latency somatosensory evoked potentials (SSPE). Thefindings indicated that the oculomotor nerve injury was located neither in the brainstem nor in or peripheral to the cavernous sinus. Clinical manifes-tations of this patient simulated those of oculomotor palsy secondary to aneurysms at the junction of internal carotid and posterior com-municating arteries. Injury of the oculomotor nerves in this patient thus seemed to be located at the site of entry of the nerves into the cavernous sinus.
