耳鳴の鍼治療—（第1報）客観的評価法による治療効果の判定

Acupuncture Treatment of Tinnitus (Report I):Evaluation of Its Efficacy by Objective Methods

はじめに

　耳鳴は，耳鼻咽喉科領域において比較的多く認められる症状の一つである。そのほとんどが当人のみにしか聴こえない自覚的耳鳴であり，大半が感音難聴に伴うものである。西洋医学領域では耳鳴治療は薬剤療法を中心に各種工夫されているが，その効果の程度，持続性，副作用等の点で未解決点も多い。一方，東洋医学領域においても既に古典「黄帝内経素問」に耳鳴は経絡の一つである少陽に関連するとの記載があり^1），鍼治療は現在なお常用されている。しかし，二重盲検法では対象群との間に有意差なしとする報告もあり^2,3），適当な治療法の確立にはなお工夫を要する段階と言えよう。そのためにはまず治療効果を客観的に捉える方法をみつけなければならない。

　一般に，感覚異常に対する鍼治療の効果の定量的判定は困難を伴うことが多く，耳鳴についても例外ではない。今回，われわれは耳鳴に対する鍼治療の効果を客観的にとらえることを目的として，いくつかの方法を導入，鍼治療の効果を判定し，その治療効果を確認することができたので報告する。

Acupunctuc has long been used for the treat-ment of tinnitus. Its efficacy, however, is stil not well established. This is partly because the evalua-tion was not done by objective methods. There-fore we applied several quantative and qualita-tive methods to evaluate it. We used question-naire and pitch-match test to evaluate the effect of acupuncurc on pitch of tinnitus. And we used visual analogue scale and digital scale and loudness-balance test to evaluate the effect of acupuncture on loudness of tinnitus. Although the results obtained by each method did not always correlate well, the efficacy of acupuncture on tinnitus was clearly demonstrated by all the methods used.
