頸部に遺残を認めた電動歯ブラシ口腔外傷の1例

A case of electric toothbrush oral trauma with cervical residuals

はじめに

　歯ブラシによる口腔・咽頭外傷は小児例が多く成人例は稀で，電動歯ブラシの使用による口腔・咽頭外傷に限定するときわめて少ない。歯ブラシによる口腔・咽頭外傷は口腔内から視認できることが多く外来で抜去可能で，ほとんどが軽症で自然治癒することが多い。受傷部位は軟口蓋，頰粘膜，硬口蓋であることが多く^1），口腔底が受傷部となることは比較的少ないとされている。今回われわれは，成人で電動歯ブラシのブラシヘッドが口腔底から頸部へ穿通し，頸部の咽頭腔外に遺残したため，頸部外切開併用で異物摘出・排膿を行った1例を経験したので，若干の文献的考察を加えて報告する。

　We report a rare case in which an electric toothbrush penetrated from the floor of the mouth to the neck of an adult patient. Due to the inability to observe the object from inside the oral cavity, outpatient removal was not feasible, necessitating surgery via a neck incision. The patient required oral nutrition and intubation due to postoperative laryngeal edema, which gradually resolved. The increasing use of electric toothbrushes in the future poses a potential for more serious oral and pharyngeal injuries. Therefore, emphasizing caution and product improvement is essential.
