 A 67-year-old woman with a history of tumor resection and anterior cervical fusion eighteen years previously for chordoma arising from the cervical spine presented with pain and weakness from the left shoulder to the forearm. Radiological studies suggested recurrence of the chordoma at the mid-cervical region. Surgery was performed via a posterior approach and after hemilaminectomy, the tumor was resected in piecemeal fashion because of its tight adhesion to and infiltration of the vertebral bodies. The results of the histological investigations were compatible with chordoma. The initial radical resection may might have allowed a lengthy disease-free period despite the high propensity for local recurrence seen in this disease.
