外科的治療が奏効した上咽頭放線菌症の1例

Successful management of nasopharyngeal actinomycosis with surgical debridement: a case report

はじめに

　放線菌症は，グラム陽性嫌気性菌であるActinomycesによって起こる感染症である。本症は頭頸部領域に好発するとされるが，上咽頭に発症した例の報告は少なく稀である。今回われわれは，上咽頭に生じた放線菌症に対し外科的治療が奏効した1例を経験したので，若干の文献的考察を加え報告する。

　We describe the case of a patient with nasopharyngeal actinomycosis who underwent trans-nasal surgery. A 48-year-old woman who complained of postnasal drip was referred to our department from an ENT clinic. Endoscopic nasopharyngeal examination revealed that she had a swollen adenoid with purulent secretion. Medical treatments, including several antibiotic therapies, were not effective. The patient underwent trans-nasal endoscopic resection of the adenoid with a microdebrider under general anesthesia. Pathological findings revealed actinomycosis in the adenoid. Her symptom disappeared after the surgery, and no recurrence was noted in her nasopharynx 7 months after the surgery.
