深頸部膿瘍の入院長期化に関する指標

Indicators associated with length of hospitalization in patients with deep neck abscesses

はじめに

　深頸部感染症は，頭頸部に存在する筋・骨・神経・血管を包む筋膜間の疎な結合織からなる間隙に生じる感染症の総称である。深頸部膿瘍はそのなかでも最も重篤な疾患の1つであるが，近年では抗菌薬や感染症治療の発展に伴い，比較的軽症で推移して退院できる症例が増加している。一方で，縦隔炎や気道狭窄，敗血症などの重篤で致死性を有する合併症を引き起こす症例も依然として存在する。

　今回われわれは，最近2年間に経験した深頸部感染症症例を対象として，入院期間が長期化する指標について検討したので文献的考察を交えて報告する。

　Deep neck abscesses are one of the most critical head and neck diseases. We analyzed the clinical factors related to the length of hospitalization in patients with this condition. The study cohort comprised 11 patients who underwent surgical drainage for deep neck abscesses between April, 2017 and August, 2019. The median length of hospitalization was 20 days（range, 8-119 days）. As a result, odontogenic infections, high CRP values（≥14.5 mg/dL）, mediastinum infections, and carotid artery space abscesses were the factors that were statistically most associated with long-term hospitalization. Our findings suggest that these cases require intensive care and an antibacterial approach.
