I．はじめに

　梅毒性内耳障害は先天性と後天性がある。その中で第3期以降の晩期梅毒による内耳障害についてはすでに多数の報告例がある。しかし第2期以前の早期梅毒による内耳障害については，最近の国内文献で江口ら^1），本村ら^2）の報告はあるものの数少ないものと思われる。今回われわれは両側の急速に進行した感音性難聴の原因として，早期後天性梅毒による内耳障害を強く疑う症例を経験したので報告する。

　梅毒は抗生物質などの発達により昭和20年代より一定した減少傾向を示しており，昭和58年の患者発生数は約1,600名である^3）。しかし症状を示さない潜伏梅毒はそれ以上の数が予想され，突発性難聴，Ménière症候群などとの鑑別上重要な疾患と思われる。

　A 40-year-old female with complaints of right ear deafness, vertigo and tinnitus. Instead of a pertinent dosis of medicaments, the deafness in the left ear and right facial palsy appeared successively. Examinations including neurological tests and lumbar puncture suggested that these symptoms were caused by the acquired syphilis. Observation of the course and survey of literatures lead us to the following conclusions.

　1. These symptoms may be caused by an inner ear disturbance and a localized meningitis due to secondary syphilis.

　2. Acute syphilitic basilar meningitis may cause a bilateral VIIIth nerve disturbance involving inner ear damage and a right VIIth nerve disturbance.
