進行性両側性難聴を呈したびまん性転移性髄膜癌腫症の1症例

A Case of Diffuse Metastatic Leptomeningeal Carcinomatosis (DMLC)

　はじめに

　突発性難聴は，比較的頻度の高い疾患であり，われわれも日常の診療の中でしばしば遭遇する。一般的には，中年に多い疾患であるとされているが，高齢者でも突然起こった片側性感音難聴が認められ，他の脳神経症状が認められなければ，とりあえず突発性難聴として対処される。しかし，確定診断にいたるには，他の疾患を除外する必要があり，時にそれが容易でない場合がある。最近われわれは，胃癌にて胃全摘の既往歴をもち，急に起こった左難聴・耳鳴で発症し，両側の難聴が次第に進行した，びまん性転移性髄膜癌腫症の1症例を経験したので報告する。

Diffuse metastatic leptomeningeal carcinomatosis (DMLC) was found in a patient with disequili-brium with a hearing loss of the left ear nineteen months after gastrectomy for stomach cancer. He was diagnosed as sudden deafness of the left ear and in spite of steroid therapy, the symptoms were aggravated without remission. After admis-sion, CSF examination revealed DMLC, and he died six weeks after his initial visit. Relatively slow progress of bilateral hearing loss without facial palsy suggested the possibility of the tumor cell invasion into the inner ear through the cochlear aqueduct.
