A case of a tracheal T-tube left unreplaced for 28 years

はじめに

　気管Tチューブ（以下，Tチューブ）は，1965年にMontgomery^1）により報告されて以降，主に気管狭窄に対するステント治療として広く使用されている。Tチューブは閉塞や感染などの合併症を考慮して，定期的に交換が行われることが一般的である。今回われわれは，28年間同一のTチューブが留置されていた症例を経験したので，文献的考察を加えて報告する。

　We report the case of a patient in whom intratracheal T-tube was left unreplaced for 28 years. Twenty-eight years ago, she underwent treatment for total anomalous pulmonary venous connection. The left lung was removed and tracheostomy was performed, which led to the formation of a tracheocutaneous fistula. Failure to replace the T tube led to sputum adhesion and caused symptoms of dyspnea. The fistula spontaneously closed without serious complications a week after removal of the T-tube. In patients with a T-tube, it is important to pay attention to the possibility of intratracheal granulation and tube blockage.
