当院で経験した中耳放線菌症3例の検討

Three cases of actinomycosis in the middle ear and mastoid

はじめに

　放線菌症は，嫌気性または微好気性グラム陽性桿菌であるActinomyces israelii（A. israelii）などのActinomyces属によって引き起こされる，慢性化膿性もしくは肉芽腫形成性の稀な疾患である^1）。Actinomyces属は口腔内に常在しており，齲歯などにより繁殖して深部組織に移行し，顔面・頸部や呼吸器に感染症を生じる^2）。放線菌症の約60％は顔面・頸部に生じるとされているが^3），稀に中耳への感染例も報告されている^4-8）。中耳放線菌症はきわめて稀な疾患であるため，これまでの報告はほとんどが1例報告であった。今回われわれは，浜松医科大学医学部附属病院（以下，当院）で経験した中耳放線菌症3例について，初診までの経過，検査所見，治療法についてまとめたので，文献的考察を含めて報告する。

　Actinomycosis is a rare, chronic and slowly progressive granulomatous disease, caused by Gram-positive, anaerobic bacteria of the Actinomyces species. The most common site of Actinomyces infection is the cervicofacial region, and actinomycosis in the middle ear and mastoid is a very rare disease. Herein, we reported three cases of actinomycosis in the middle ear and mastoid, diagnosed from the histopathologic features of specimens collected from middle ear. Difficulty in diagnosing actinomycosis by bacterial culture of the specimen was attributable to the fact that Actinomyces species are hard to grow on agar plates.
