Ⅰ　はじめに

　悪性外耳道炎は，外耳道の細菌感染が軟部組織や骨・軟骨に壊死性・破壊性に進展し，側頭骨の骨髄炎を引き起こし，時に致命的となる疾患である。本疾患は，高齢の糖尿病患者に多く，患者は頑固な耳痛と耳漏を伴い，起炎菌のほとんどが緑膿菌である。近年，緑膿菌に有効な抗菌薬の発達により死亡率が減少するとともに，多くの症例が保存的治療により治癒に至ることが報告されている^1）。一方，悪性外耳道炎は緑膿菌以外の起炎菌によって発症することもあるが，症例数が少ないため，その実態は不明な部分が多い。今回，われわれは耳漏よりメチシリン耐性ブドウ球菌（methicillin-resistant Staphylococcus aureus：MRSA）が検出され起炎菌と考えられた悪性外耳道炎症例を経験したので，文献的考察とともに報告する。

　Malignant external otitis is an invasive infection of the external auditory canal and affects elderly patients with diabetes mellitus. An 85-year-old male with diabetes mellitus was referred to our hospital with complaints of left ear pain and discharge. On examination,the inferior and posterior walls of the left external auditory canal were destroyed and cultures of aural drainage demonstrated methicillin-resistant Staphylococcus aureus. Malignant external otits was diagnosed,and the patient was treated with vancomycin. Because ear discharge could not be controlled despite the treatment,tympanoplasty with mastoidectomy was performed. His postoperative course was satisfactory and there was no sign of recurrence.
