 A 81-year-old woman underwent tracheostomy under general anesthesia for dyspnea. She had had ALS 2 years before. After the tracheal tube was inserted, the patient's lungs suddenly could not be ventilated. SpO2 decreaced to 43%. Subcutaneous emphysema on the right side appeared. A chest X-ray revealed subcutaneous emphysema, mediastrial emphysema, and right pneumothorax. It was speculated that a proximal tracheal obstruction and high-pressure ventilation air caused a mediastrial emphysema and rupture of the mediastrial pleura, resulted in pneumothorax.
