 The patients with oblique nose usually have a deviated nasal septum.

 The simultaneous operation of nasal bone osteotomy and reconstruction of nasal septum is thought to have a risk of causing saddle nose. Especially, it is difficult to correcting a frontward deviated nasal septum by Killian's submucosal incision technique.

 A 17-year-old man complained of nasal obstruction and oblique nose with frontward deviated nasal septum. We performed simultaneous operation of nasal bone osteotomy and reconstruction of septal cartilage for correction by applying a method of hemitransfixion approach. Postoperatively, a good aesthetic effect was provided and nasal obstruction markedly improved without causing saddle nose.
