外科的介入を行った直腸癌甲状腺転移の1例

A case of thyroid metastasis from rectal cancer

はじめに

　直腸癌の他臓器への転移は首位が肝臓（直腸癌全体の9.5％），次いで腹膜（3.0％），肺（1.7％），骨（0.3％），脳（0.1％）と続く^1）。治療はおもに外科的治療および化学療法，放射線療法を症例に応じて選択し，原発巣および遠隔転移巣がともに切除可能な場合には特に外科治療を考慮する。肝転移に対しては熱凝固療法も選択可能である^1）。これら直腸癌遠隔転移例の治療後の予後は，肝切除後の5年生存率が20〜50％，肺切除後の5年生存率が30〜60％と比較的良好である^2,3）。

　一方，直腸癌の甲状腺への転移は上記に比べ稀とされていたが，近年頭頸部画像診断の進歩により報告例が増加している。しかし手術適応に関してはいまだに一定の見解が得られていない。今回われわれは直腸癌の甲状腺転移の1例を経験したので文献的考察を加えて報告する。

　A 51-year-old male who had received resection of rectal cancer 4 years ago, and heavy ion radiotherapy for following local recurrence, partial resection of lungs, transarterial chemoembolization, chemotherapy for following pulmonary metastases, was detected the accumulation in the right robe of thyroid glands with methionine-positron emission tomography. He was diagnosed as thyroid metastasis from rectal cancer, and right lobectomy was performed to prevent dysphasia and asphyxia.

　In Japan, 26 cases of thyroid metastasis from rectal cancer had been reported. They increased especially since 2005, mostly followed pulmonary metastases. In conclusion, thyroid resection is beneficial in preventing laryngeal trouble when other metastases are settled.（100 words）
