頭蓋骨を広範に破壊した悪性線維性組織球腫の1例

A Case of Malignant Fibrous Histiocytoma (MFH) with Extensive Destruction of the Skull

はじめに

　悪性線維性組織球腫（malignant fibrous histio—cytoma, MFH）は，組織球由来の細胞が繊維芽細胞に変化する腫瘍で，多核巨細胞，炎症性細胞等を伴い，極めて多彩な像を呈するものである^1）。その病理学的診断は必ずしも容易ではないが，いわゆる花籠状^2）（storiform pattern）を呈することを特徴としている。MFHは軟部組織に発生する成人の悪性腫瘍の中ではもっとも多く^3），ほぼ全身に発生することがわかっているが，近年骨由来のMFHも報告されている。

　他の悪性腫瘍と同様に，臨床像は局所に腫瘤を形成しつつ周囲を破壊，増殖していくものであるが，頭頸部MFHの場合，局所再発，遠隔転移の頻度が高く，予後不良である。

A case of malignant fibrous histiocytoma origi-nated from the maxilla is reported. A thirty-two-year-old female underwent nasal polypotomy. The histological examination revealed benign osteogenic tumor. Thereafter, destruction of the facial and cranial bones developed resulting in a deformed face.

Diagnosis of MFH of myxoid type was finally made after the repeated biopsies. Partial resection followed by radiation therapy provided little im-provement. Subsequently the patient died. Al-though this tumor extensively invaded the bone, the sorrounding soft tissue remained intact except the forehead and the palatinum where the perio-stium was damaged by biopsy.
