Two cases of laryngeal sensory neuropathy

はじめに

　耳鼻咽喉科日常診療において，咽喉頭異常感症や遷延する咳嗽は比較的よく遭遇する症候であるが，治療に難渋する症例も稀ではない。最近，海外において，咽喉頭異常感症や遷延性咳嗽の原因として，迷走神経障害に起因する病態が考えられており，laryngeal sensory neuropathy^1,2）やpostviral vagal neuropathy^3）の名称で報告が散見される。これらの治療として，抗うつ薬や抗けいれん薬が有効である。しかしながら，いまだわが国での報告はない。今回，laryngeal sensory neuropathyと考えられ，神経障害性疼痛治療薬であるプレガバリンが奏効した2症例を経験したので報告する。

　Chronic cough and throat discomfort are common symptoms. In spite of an extensive workup and empiric treatments, not a few patients continue to have these symptoms. Recent literature has shown laryngeal sensory neuropathy（LSN）is one of the causes of these symptoms and can be treatable with neuromodulator medications.

　I experienced two cases of LSN. The first case was a 74-year-old male who complained of odynophagia for one month and the second was a 40-year-old female who complained of cough for two months. After treatments of regular regimen without relief, pregabalin was used in both cases and their complaints were disappeared.
