鼻腔悪性黒色腫に対するDAC-Tam療法施行中に発症した急性心筋梗塞例

A case of nasal mucocutaneous melanoma developed acute myocardial infarction related to chemotherapy（DAC-Tam）

はじめに

　悪性黒色腫において，化学療法は外科的切除の適応がない進行症例や外科療法後の術後補助療法として用いられている。

　近年，悪性黒色腫に対する化学療法としてわが国で最もよく行われているレジメンはDAC-Tam療法である^1）。元となったのは，Dartmouth医科大学のDel Preteら^2）が1984年にDartmouth regimenとして報告した治療法で，cisplatin（CDDP），carmustine（BCNU），dacarbazine（DTIC）に抗エストロゲン薬であるtamoxifen（TAM）を加えたものであった。わが国では山﨑ら^3）が未承認薬であるBCNUをnimustine（ACNU）に置き換えてDAC-Tam療法として施行し，それ以前からよく施行されていたDAV-feron療法と比較して良好な成績を報告している。

　一方で，このレジメンに使用されている抗がん薬では副作用の1つとして心毒性が知られている。抗がん薬による心毒性は血液毒性や消化管毒性などと比べるとその発生頻度は低いものの，ほとんどの抗がん薬が心毒性をもつといっても過言ではない^4）。

　今回われわれは，鼻腔悪性黒色腫に対してDAC-Tam療法を施行中に急性心筋梗塞（AMI）を発症した症例を経験したので，文献的考察を加えて報告する。

　A 68-year-old man with nasal mucocutaneous malignant melanoma suffered from acute myocardial infarction（AMI） during chemotherapy. He developed AMI on the 11th day of the third chemotherapy course with conventional dose of DAC-Tam. This patient had history of hypertension and hyperlipidemia, but they were well controlled. Some cases of AMI induced by cisplatin or tamoxifen have been reported. We suspected that the case of AMI was closely associated with chemotherapy, especially tamoxifen, since this patient developed AMI after extending the dosing period of tamoxifen, and AMI was caused by coronary embolism. Chemotherapy should be used cautiously in patients with coronary risk factors.
