対側の舌下神経麻痺を含む多発脳神経障害をきたしたハント症候群の1例

Ⅰ　はじめに

　ハント症候群は耳性帯状疱疹，末しょう性顔面神経麻痺，内耳障害を3徴とする疾患であり，一般にその病態は第Ⅶ脳神経の膝神経節に潜伏する水痘帯状疱疹ウイルス（VZV）の再活性化と考えられている。

　ハント症候群は第Ⅶ，Ⅷ脳神経障害を主とする疾患であるが，Bell麻痺（BP）との鑑別が困難とされる耳性帯状疱疹を伴わないハント症候群（zoster sine herpete：ZSH）を経験する一方で，比較的稀ではあるが下位脳神経を中心とした他の脳神経障害を合併する症例もみられる。

　今回われわれはハント症候群による第Ⅶ，Ⅷ脳神経障害に加え，患側の第Ⅸ，Ⅹ，Ⅺ脳神経および対側の第Ⅻ脳神経障害を認めた症例を経験したので，文献的考察を加えて報告する。

　Ramsay Hunt syndrome（RHS） has been thought to be caused by a reactivation of varicella-zoster virus in the geniculate ganglion,and it could be characterized by peripheral facial palsy,herpes zoster oticus and inner ear symptoms. On rare occasion,lower cranial nerve involvements occur in RHS in addition to these symptoms. We presented a case of woman with RHS with involvements of ipsilateral Ⅶ,Ⅷ,Ⅸ,Ⅹ,Ⅺ and contralateral Ⅻ cranial nerves. This is a rare case because there was only one report of RHS not only with ipsilateral cranial nerve involvement but with contralateral Ⅻ one.
