敗血症ショックを生じた急性喉頭蓋炎の1例

Acute epiglottitis complicated by septic shock：a case report

Ⅰ　はじめに

　急性喉頭蓋炎はときに急激な気道狭窄をきたすことがあるため，局所の気道管理がきわめて重要である^1）。しかし局所病変だけでなく，全身状態の把握が重要で，重篤な合併症を伴う症例では迅速な対応が必要とされる^2）。

　今回われわれは敗血症ショック，ならびに急性呼吸促迫症候群（acute respiratory distress syndrome：ARDS），播種性血管内凝固症候群（disseminated intravascular coagulation：DIC），多臓器不全（mutiple organ failure：MOF）を生じた急性喉頭蓋炎を経験したので若干の文献的考察を加えて報告する。

　Acute epiglottitis causes sudden upper airway obstruction that may be fatal. So,airway management is very important for acute epiglottitis. But,few patients suffer associated extraepiglottic complications. We present a 42-year-old man with acute epiglottitis complicated by septic shock,which is very rare,and only 1 case have been described to date.
