喉頭に発生した尋常性疣贅の2例

Ⅰ　はじめに

　尋常性疣贅は通常，四肢末しょう皮膚に発生する表面乳嘴状の角化性丘疹で，主にヒト乳頭腫ウイルス（HPV）2，4，27，57型の感染により生じる^1,2）。これらのHPVは微小な外傷から侵入し，1～6か月間の潜伏期を経て発症するとされている。頭頸部領域ではHPVに関連した疾患として喉頭乳頭腫や中咽頭癌が知られているが，粘膜を発生母地とした尋常性疣贅はきわめて稀で，口唇，舌および喉頭に発生した症例が少数報告されているにすぎない。今回われわれは声帯に発生した尋常性疣贅の2例を経験したので文献的考察を加えて報告する。

　Verruca vulgaris（VV）of the larynx is a rare lesion related to the human papillomavirus（HPV）. A 67-year-old female underwent transoral surgery for the treatment of VV in the right vocal cord. Two years later,another lesion was observed in the same location. Pathological diagnosis of the second lesion was squamous cell papilloma. HPV typing using Multiplex PCR detected HPV 6 in both lesions. A 60-year-old man had CO₂ laser surgery for the treatment of the lesion in the left vocal cord. Pathological diagnosis of the surgical specimen was VV,but HPV 6 was negative.
