本態性音声振戦症に対する芍薬甘草湯®の使用経験

Preliminary study on administration of Chinese medicine；Syakuyaku-Kanzoutou® for essential voice tremor

Ⅰ　はじめに

　本態性音声振戦症は，発声時にのみ両側声帯，咽頭壁，軟口蓋などに4～5Hzの周波数の律動的な相反性反復運動が出現し，声の震えを生じる疾患であり，動作性振戦症の一亜型として錐体外路系疾患に分類される^1）。その原因の詳細については明らかになっていないが，大脳基底核に原因病巣を求めるとする説が有力である^1）。治療方法として，β遮断薬や抗痙攣薬，抗不安薬などによる薬物治療^2,3）のほかに，音声訓練^4～6），ボツリヌス毒素の甲状披裂筋内注入^3,7,8），深部脳刺激療法^9,10）などの報告があるが，有効で，かつ副作用が少ない治療法は確立されていないのが現状である^3）。また，本態性音声振戦症の好発年齢は50歳以上，特に70歳以上の高齢発症例の多いことが報告されていることから^3,11,12），治療を行うに当たっては，既往疾患や治療による合併症についても十分検討を行う必要があると考えられる。一方，最近，本態性振戦症に対して漢方製剤である芍薬甘草湯^®が有効であったとする報告^13,14）があるため，今回，われわれは本態性音声振戦症の3症例に対して，芍薬甘草湯^®を投与し，その効果につき検討を行った。

　We administrated Chinese medicine called Syakuyaku-Kanzoutou^® for three cases with essential voice tremor.

　Syakuyaku-Kanzoutou^® is a Chinese medicine that could accomplish immediately to mitigate the muscle spasm and its related pain regardless of skeletal muscles or smooth muscles,and used for cramp in the calf,hiccup,celialgia,tension headache and others. In 2 cases（a 76-year-old woman and a 75-year-old man）,their symptoms were improved with decreasing of the frequency of the voice tremor and elongation of the phonation time in one cycle of the voice tremor. In a 71-year-old woman,her voice tremor was not improved.
