Ⅰ．はじめに

　上皮小体癌は全悪性腫瘍のうち0.005％と稀な疾患であり，米国における罹患率は人口100,000人当たり0.015人で，有病率は0.005％と推定されている^1～5）。原発性上皮小体機能亢進症のなかでの割合は0.6～4％程度である^6）。一般に腺腫や過形成と比較して大きいものが多いが，50mmを超えるものは10％程度で，血清Ca値は14mg/dl以上と著明な高値を示す場合が多い^7～8）。

　今回われわれは血清Ca値がほぼ正常上限の巨大な上皮小体癌を経験したので報告する。

　A parathyroid carcinoma is a very rare tumor and mostly accompanied with hypercalcemia but no high value of Intact-PTH. We reported a rare case of giant parathyroid carcinoma without hypercalcemia and the high value of Intact PTH. The differential diagnosis of the rapid growth of an anterior neck mass should be considered as parathyroid carcinoma.
