急速に増大して治療を要した乳児喉頭囊胞の1症例

An infant with laryngeal cyst

Ⅰ．はじめに

　成人の喉頭囊胞は耳鼻咽喉科外来診療においてしばしば目にする疾患であり，無症状で経過する場合が多いため，臨床的にあまり注目されていない。一方，新生児ならびに乳児における喉頭囊胞は頻度としては稀であるが^1），時に呼吸困難や嚥下障害をきたし，死に至る可能性もある。これは，成人と比較して新生児，乳児では気道が狭いこと，喉頭組織が脆弱であり^2），囊胞により圧排されやすいことなどが原因であると考えられる。Suehsら^3）は過去の報告例のうち約半数は窒息死後の剖検によって診断されたと述べており，喉頭囊胞は乳幼児の突然死の原因としても看過できない疾患である。

　今回われわれは喉頭脆弱症に急激な増大をきたす喉頭囊胞を合併した生後2か月の症例の喉頭囊胞摘出術を経験したので，特に手術適応および手術時期の決定に関して考察を加えて報告する。

　Laryngeal cyst is uncommon disease in infants, however it rarely causes dyspnea and dysphagia and lead to mortality. The proper management of the infant laryngeal cyst is thus important. A 2-month-old female infant visited our clinic with stridor and poor feeding. Flexible laryngoscopy revealed laryngomalacia and a small cyst in the vallecula. Although we had waited laryngomalacia to improve spontaneously, the cyst grew up rapidly and her larynx collapsed severely. We therefore resected the cyst under general anesthesia and the symptoms disappeared immediately. We discuss the management of infant laryngeal cyst.
