 Adenoid cystic carcinoma is a common salivary gland tumor, but rarely arises from the skin. A 74-year-old woman had been aware of a mass in her left temporal scalp for 9 years. Incision of the mass was performed by a previous physician, and histopathological diagnosis was adenoid cystic carcinoma. She underwent wide resection of the tumor followed by reconstruction with a local skin flap. Two years later, ipsilateral cervical lymph node swelling occurred, and the patient was referred to our department. Computed tomography, magnetic resonance imaging, and positron emission tomography revealed metastatic lymph nodes in the posterior and upper cervical regions. Conservative neck dissection was performed, and the histopathological examination of the surgical specimen confirmed lymph node metastasis of adenoid cystic carcinoma. The patient's postoperative course was uneventful. She has been free from disease 5 months after neck dissection.
