喉頭内転術2年後にシリコンプレートが脱落した1例

Extrusion of a silicon plate 2 years after thyroplasty

Ⅰ．はじめに

　甲状軟骨形成術Ⅰ型は片側の声帯麻痺などによる声門閉鎖不全で生じる嗄声の改善手術としてIsshikiら^1）が発表した，喉頭内転術（声帯内方移動術）の1つである。この方法は声帯のレベルで甲状軟骨を開窓して，そこから形をトリミングした軟骨やシリコンプレートを挿入して声帯を内方に移動させるもの^2）である。効果が確実なことや手術が比較的容易なことなどから広く普及している。手術法も原法と少しずつ異なった方法が行われており，それに伴い何らかの合併症も生じてくることが考えられる。今回われわれは喉頭内転術後にシリコンプレートが喉頭内に脱落した1例を経験したので報告する。

　A 80-year-old woman presented with hoarseness. A silicon plate was extruded under the vocal cords 2 years after thyroplasty type Ⅰ. It was shown that the extruded silicon plate become an airway foreign body. To avoid extrusion the silicon plate after thyroplasty type Ⅰ we should pay attention to the way of fixation of the plate and position of the fenestration on the thyroid ala.
